Labs not for ESRD Treatment

Labs that are not for the treatment of ESRD must have appropriate medical necessity documentation.  For clinical labs, appropriate medical necessity documentation includes:

  • Clinical justification from the physician for the lab order
  • A signed lab order by the physician
  • Documentation that:
    • The result was reviewed by the physician; and
    • How the lab result was used in the treatment of the patient’s condition

The twelve lab tests that require this additional information include: ALT / SGPT, HGB A1c, TSH, Cholesterol, Triglycerides, HDL, LDL, Hep C Ab Reflex, RNA Hep C, SARS-COV2 RTPCR, SARS-COV2 IgG, and SARS-COV2 IgM.

At the top of the screen, the number of total patients with labs not for the treatment of ESRD and the time and date the data was last updated is available.

Data for the patient includes last name, first name, age, gender, facility name, lab test name, lab test result, and lab draw date.  A Past Due indicator will display if the lab result has not been reviewed within 30 days of the resulted date.

Then you hover your cursor over the lab value, a tooltip will display the lab test name, result value, reference range, collection date, resulted on date, and test code.

The lab results in the queue must be reviewed and indicated with either a “Reviewed, No Change [in Care Plan]” or “Reviewed, Changed Care Plan” for each test result.  In addition, a Comment box is offered to give physicians the ability to document how labs ordered not for the treatment of ESRD were used in the treatment of the patient’s condition.  Please note that any changes to the care plan must be directly communicated to the DaVita Care Team.


To document review of non-ESRD labs:

Step 1: From the OneView home page, click the “Labs Not for ESRD Treatment” task card

  • ALTERNATE OPTION: From the left navigation menu, select TASKS > Labs Not for ESRD Treatment or via the encounter note in the patient’s chart.

Step 2: Select the radio button associated with the applicable response for each lab result

NOTE: Select the “Select All To Reviewed, No Change” button to designate all labs as “Reviewed, No Change”.  Any lab designation selection can be modified to “Reviewed, Changed Care Plan” any time before clicking the “SIGN and SUBMIT” button.

TIP: If you select “Reviewed, Changed Care Plan” a comment box will automatically open.  Please note that any changes to the care plan must be directly communicated to the DaVita Care Team.

Step 3: Enter a reason for the change in the care plan, if applicable

Step 4: Click SIGN AND SUBMIT to submit your information to DaVita labs

NOTE: The total number of lab results that require documentation will decrease by those acted upon.  If the documentation is completed in OneView mobile, the lab result will not be presented again for documentation in OneView and vice versa.

Non-ESRD comments from OneView and OneView Mobile will display in the appropriate care category of the patient's chart under the ESRD/AKI Mgmt tab after the provider finalized and submits the attestation.  Data for the comment includes comment entered date, lab test name, lab test result, lab draw date, attestation, attestation comment, and the provider that entered and submitted the attestation.

The document icon is shown for the non-ESRD lab comments documented in the encounter note.  Click the document icon above the care category comment and a separate tab will open with the full encounter PDF.

Non-ESRD comments documented in the Labs Not For ESRD Treatment queue contain the test tube icon at the top of the data.
