Complete Influenza Attestation Form Job Aid

This job aid will guide providers through the process of completing the influenza vaccination attestation form in OneView or OneView Mobile.  

Complete Flu Attestation in OneView

STEP 1: Go to oneviewdps.davita.com

STEP 2: Log in with your DaVita username and password

STEP 3: Locate the Flu Attestation Form in the My Activities & Reminders section of the Home Page

STEP 4: Click the influenza vaccination attestation message to open

STEP 5: Select the radio button for the option that applies to you

NOTE: If you’ve received a vaccination from an outside location, you will be required to indicate the location and date

STEP 6: Click the Acknowledge Here checkbox

STEP 7: Click Submit

Complete Flu Attestation on OneView Mobile

STEP 1: Tap the OneView Mobile app

STEP 2: Log in with your DaVita username and password

STEP 3: Locate the applicable influenza vaccination attestation message in the COMMs Hub Feed on the Home page or tap the Menu icon to select Comms Hub

STEP 4: Tap the Flu Form to open

STEP 5: Select the radio button for the option that applies to you

NOTE: If you’ve received a vaccination from an outside location, you will be required to indicate the location and date.

STEP 6: Tap the toggle to acknowledge

STEP 7: Tap Submit
