Care Activity and Patient Notes

The care activity and patient notes finalized in CWOW by the care team within the last 6 months are now available in the Clinical Details tab of the patient’s chart.  They display within the notes carousel in the associated care category, along with the provider notes.  The care activity and patient notes may contain a title, completed date, comments or details, and care category tags.  DaVita-approved education materials provided to the patient are documented with the associated care team note.

The Mandatory Reporting of Abuse or Neglect assessment is only visible to providers and will have a Sensitive information indicator.  Practice staff will not be able to view this assessment.
