IKC Hospitalization Alert

The Focus Patient Indicators banner appears in the Patient Status section of the patient's chart when they have an IKC Risk flag or have been hospitalized in the last thirty days. The banner contains the core team and recent hospitalization, if available.  The core team record displays the risk rating and reason.  The recent hospitalization data shows the reason for admission, length of stay, admission and discharge dates, and primary and secondary diagnosis.
