Patient Chart

From the patient search, tapping the patient’s name will open their chart.  All data displayed is read-only.  To add, update, or remove any of the data under any area of the patient’s chart, the patient’s DaVita care team must be contacted.


Demographics contain the most recent patient and dialysis facility details.  The information is read-only.  Tapping the telephone icon will prompt you to dial the phone number listed.

Tapping the white arrow at the top of the screen will return you to the Patient Summary.

Hemodialysis Treatments

Hemodialysis Treatments will display read-only data of the patient’s current treatment details.  The last treatment date, dialysate/sodium data, flow rates, ultrafiltration, instructions for use, and Heparin are available.


Encounters provide a list of the patient’s limited and comprehensive encounters, created in OneView, along with the provider who created the encounter and the creation date.   Tapping an encounter will display the contents of that encounter. The information is read-only.


Labs contain read-only data for the patient’s current and historical lab results.  Up to 12 months of historical lab results will be displayed.  Lab details are organized by category - General and Dialysis.  Tapping the lab sub-category will open the patient’s lab results.  Scroll to the right to view more results or turn the mobile device horizontally to view in landscape mode. 

An indicator of H (High) or L (Low) will display to the right of any lab that is out of range.  High/Low lab values will also be displayed in red so they stand out to the user.  A dash indicates that there is no lab value for that date.

The long press (haptic touch) function can be used to view additional information about any lab value.  Touch the result with your fingertip and hold it there for a few moments.  The mobile device will vibrate and the test name, result value, test code, reference range, collection date and time, and laboratory name will appear.  Only partial data will be viewable on initial load; scroll down to view all the information.  Tap anywhere on the screen to return to the prior view.

BP & Fluid Management

BP & Fluid Management contains the patient’s current and historical blood pressure readings and fluid-related labs.  Thirty days’ worth of the most common data such as target weight, treatment duration, and blood pressure is available.


Problems display the patient’s documented problems.  The information is read-only.  Tapping a problem will display the ICD 9 and 10 codes for the selected problem.


Medications display the patient’s documented medications.  The patient’s active medications list from Snappy and MedCheck is the default display.  The medication categories are:  Medications, Prescribed, and Home.  The Medications list will include current treatment and home medications. The data displayed is read-only. 


Allergies include a list of the patient’s active allergies.  The list will be sorted by severity, with the most severe allergies listed at the top.

Tap the allergy name to view more details about the allergy, such as type, severity, and source.

“No Allergies have been documented for this Patient” displays if no allergy information has been entered for the patient.  “No Known Drug Allergies” appears if no known allergies to medications have been entered for the patient.  Non-drug allergies will display if entered.


Cultures has up to one year of microbiology results. The culture name, final result, and date are listed.  To view the full details of the microbiology result, tap on the appropriate result and the microbiology final report opens.


Vaccinations display the patient’s history of immunizations administered in a DaVita facility in the past two years. The vaccine name and date given will display with the most recent listed first.  “No Vaccinations have been documented for this Patient” appears if no vaccines have been entered for the patient. 


Access includes details regarding the patient’s dialysis access.  This section contains the primary and secondary access, if available, and displays the type, site, and placement date.
