Overview

ePrescribe provides the ability to create and send e-prescriptions to an outside pharmacy for non-treatment medications that are not dispensed and/or administered in the facility for Snappy and CWOW patients.

The prescriber must be a credentialed provider which means the DaVita credentialing department has given the provider privileges to provide care in a DaVita dialysis facility.

Providers that are credentialed with DaVita will be set up as prescribers in Rcopia (Dr. First). There is no covering rights functionality in ePrescribe for providers at this time. You will be responsible for creating, reviewing, signing, and sending your prescriptions.

The Rcopia application is used as the Surescripts certified application and is integrated with OneView for the purpose of ePrescribing and responding to renewal requests from pharmacies. OneView has no direct interaction with pharmacies regarding prescriptions/refills/renewals.

Prescriptions created in OneView will not be sent or appear in CWOW. Prescriptions created in OneView can only be viewed on the OneView medication list.

Limitations

Supplies, compound medications, and medications that are controlled substances (Schedule II through V) on the federal or any state level will not be allowed to be prescribed within OneView. 

Controlled substances, supplies, and compound medications will be blocked and cannot be prescribed. If a supply or compound medication is selected, a message will appear stating, “This medication/supply cannot be e-prescribed at this time in OneView. Please go back and select another medication.”

Controlled substances cannot be searched at all in the medication search.

Process

Signing is automatically done when you select send as the action for the new prescription. No actual separate signature process is required. Prescriptions can be entered and initiated in OneView or Rcopia.

Location in the App

Creating an electronic prescription is accessible in two locations within OneView. The functionality of this feature is the same in both areas.

The +ADD RX link is available via the MEDICATIONS button in the Patient Care > Clinical Details section of the chart and within the Prescriptions section under Clinical Summary.

From Rcopia, new prescriptions can be created by using the Create New Rx button or favorites list.

Prescription Statuses

Medications that are prescribed in OneView or Rcopia will display on the patient medication list in OneView with an Rx next to the medication name.

Unsigned prescriptions are indicated with an orange Pending as the medication status and are listed at the top of the Active medication list. The medication status may take up to five minutes to update.  The pending prescriptions will be listed from newest to oldest.

The All medications list will be listed in alphabetical order and show up to one year of data of active and inactive medications from Snappy, as well as, those prescribed in OneView.

Medications that were entered into CWOW by a DaVita care team member is denoted with the order type next to the medication.

Prescriptions initiated from CWOW by a care team member displays on the OneView medication list with the “Rx (Care Team)” tag next to them.  Providers must review these prescriptions on Rcopia as they currently do. Once signed, the CWOW-initiated medication is linked to the home medication order.

Finalizing Prescriptions

After the prescription is created, it must be reviewed, signed, and action must be selected to transmit to the pharmacy.

The unsigned prescription for a specific patient can be found under Clinical Summary located in the Prescriptions section or Menu > Tasks > eRX Pending to view all patients with an outstanding prescriptions.

NOTE: Prescriptions that have not been sent to the pharmacy yet can ONLY be edited in the Rcopia screen.

IKC (Integrated Kidney Care) Hospitalization Risk Rating

A new hospitalization risk badge has been added to the Patient Chart, Dialysis Schedule, Current and IKC Patients list. The 1-5 ratings are documented in CWOW and based on the Core Team’s analysis of whether or not the patient is at risk of hospitalization within the next 30 days.

The risk rating data definitions:

•Rating 4-5: The team is concerned or very concerned

•Rating 2-3: The team has some concern

•Rating 1: The team has no concern

IKC Hospitalization Alert

The Focus Patient Indicators banner appears in the Patient Status of the patient’s chart when the patient has an IKC Risk flag or has been hospitalized in the last thirty days. The banner contains the core team and recent hospitalization, if available. The core team record displays the risk rating and reason. The recent hospitalization data shows the reason for admission, length of stay, admission and discharge dates, and primary and secondary diagnosis.

IKC Patients Tab

Advance Practice Providers (APP) will have access to the IKC Patients tab in the Patients menu, which provides the patients participating in an IKC program. Data for the patient includes last name, first name, IKC badge, age, DOB, gender, phone number, and IKC program name. The list can be filtered by IKC program, patients (all, my, and covering), and modalities. Click on the patient’s name to open the patient’s chart. The APP will have the same functionality as the physician except for the DOWNLOAD MY IKC PATIENTS option.

Prior Authorization

Daprodustat was added to the list of medications that may require prior authorization. The MEDICATIONS ELIGIBILITY button under the Profile tab of the patient’s chart in the Insurance Details section contains the “Auth First” medications and coverage information.

(MOBILE ONLY – iOS and Android) Limited Notes

Limited Encounter notes are now available to all providers and can be accessed from the “Note” option in the right corner of the patient chart or the blue plus icon in Dialysis Schedule. The Limited note has the same functionality as the Comprehensive note.

(MOBILE ONLY – iOS and Android) IKC Filter

Providers can filter the Patients list by IKC Patients and IKC Program by clicking the filter icon. The IKC Patients filter contains My IKC Patients and Covering IKC patients. IKC Program will display the IKC programs in which your patients are participating.
