The care categories in the Patient Care > Clinical Details section of the chart utilizes badges and icons to communicate important or additional information for an individual data point.

Anemia

Daprodustat HIF Medication Eligibility

The Daprodustat HIF Medication Eligibility icon under the Anemia care category in the patient's chart's Clinical Details tab displays coverage information.

Click the icon to view whether a patient's insurance reimburses for the medications and if prior authorization is needed before the provider places an order.

Medication eligibility data is pulled from DaVita's Pharos and Nautilus systems. The data includes the Payor Coverage Eligibility, Payor Prior Authorization (PPA), and Pharos FE Request Status. 

Additional Daprodustat eligibility information can be found under Medication Eligibility link in the Profile tab of the patient's chart.

Mircera Info

The Mircera alert under the Anemia care category in the Clinical Details tab of the patient’s chart will calculate and display the estimated week for the next dose of Mircera when in the MEDS view.

Click the Mircera information icon, and a message appears with details about the patient’s next Mircera dose and an email address for questions or comments.

NOTE: This is based on the order frequency and date of the last Mircera dose delivered.

If the date for the patient’s next dose of Mircera is not available, the information icon will denote that.

SHAPE Trends

Under the Anemia care category of Clinical Details, a graph is available for patients that are on a SHAPE Anemia Management Protocol. The graph includes data for the SHAPE 10 protocols. To expand and view the graph, click the “+ SHAPE TRENDS”.

SHAPE 10 displays Hemoglobin (Hgb) levels and Epogen doses for patients on the SHAPE 10 Anemia Management Protocol. Up to 180 days of data will load. The graph includes the lab results, SHAPE 10 protocol suggested dose (coming from the third-party vendor - DOSIS), the prescribed dose of Epogen, missed treatments due to hospitalizations, and missed treatments. To view all data for the protocol, select “LOAD ALL DATES” located at the bottom right of the graph.

The Shape 10 protocol start date displays with a vertical line.  A blue background on the graph indicates that the patient is on the Shape 10 protocol.  A white background indicates a time before the patient was on the protocol or after the protocol was discontinued.   

Hover over any data point to obtain more details.  A blue dot indicates the SHAPE 10 protocol suggested dose from DOSIS.  If the suggested dose is overridden, the blue bar will display either higher or lower than the dot on the next subsequent treatment.  Overlapping blue dosage bars signifies consecutive dates.

If the patient is within their first 3 months on the Shape 10 protocol, an alert message will display indicating they are within this initial transition period.  Hover over the “SHAPE 10.0 START” line to display the date when the protocol began.

To obtain additional information about the protocol, click the “read more” link located above the graph.  When the “read more” link is clicked, a new browser window will open requiring the user’s AD credentials for OneView.  After the login information is entered, the SHAPE 10.0 Physician FAQ (ICHD) information will display.  To collapse the menu, click “Close”.  To return to the OneView Home Page, click “OneView App”. 

Any Care Category

Home Education Point of Care Support Articles

Point-of-care support indicators will appear on the patient chart for Home patients with specific clinical criteria.  When a support trigger is identified for a patient, a purple lightbulb icon will appear next to the care category name.  The triggers are based on specific terms documented in the encounter notes, hospitalizations, and CVC access use for PD and HHD patients.

The PD and HHD patient charts will be scanned for up to 90 days to identify these specific terms and events. 

Click on the purple light bulb icon, and a pop-up appears with the related article link.  The article will have a bookmark icon allowing you to save the articles for quick reference in the future.  The bookmarked icon turns blue if clicked.  To remove the bookmark, click the bookmark icon again.  The bookmarked articles are available in the Home Education hub.

To view the article in PDF format, click the article name. 

After viewing the article, click on the patient’s name to return to the patient’s chart.

Labs Out of Range

An indicator of H (High) or L (Low) will display to the right of any lab that is out of range. High/Low lab values will also be displayed in orange to stand out to the user.

An orange dot to the right of a lab value indicates multiple results for this test from the same draw date. When you hover your cursor over a lab value, a tooltip will display the result value, test code, reference range, collection date and time, and unit of measure. The collection time for a lab result is determined by the time zone set in your internet browser.

A comment bubble appears on the left side of a lab result whenever there is a comment or note associated with a result.

Clicking the comment bubble opens a window that displays the related comments and/or notes for the lab result.

Comments come from the laboratory (DaVita Labs). Manually entered labs will contain the result source.

The “Show More” option with a count of additional parameters in parenthesis will be available to display the remaining values for the care category.

A high/low indicator will be next to the “ Show More” if there are lab results within the past 30 days that have a high or low value hidden from the primary view. To expand the view for the remaining lab data, click the “ Show More” option. To collapse the view, click “ Show Less”.

When you hover your cursor over the data point, a tooltip appears that contains the data and treatment date. The current Prescribed Duration and Target Wt appear within the dark blue bar. The light blue bar reflects any prior Prescribed Duration or Target Wt data.

Patient Status and/or Fluid/BP

IDT Assessments

The patient’s Modality Choice Assessments, Interdisciplinary Team (IDT) Assessments, and Plan of Care (APOC) are available in the Clinical Details and Clinical Summary tab of the patient’s chart.

In the Clinical Details tab, IDT APOC documents finalized in the past 90 days are available in the Patient Status and Fluid/BP care category sections.

Unviewed IDT content will have an “Unread” indicator. Once the content is viewed, the “Unread” indicator will be removed.

Some types of assessments that may be available are: Verify Access Placement, Tuberculosis Risk Appraisal Questionnaire (TB-RAQ), Diabetes Assessment, HHD Loss, Mandatory Reporting of Abuse/Neglect, Fall Risk, Target Weight Assessment, Patient Modality Decision Follow-Up, Home Modality Education, Modality Choice Discussion, and Administer PRAPARE (Protocol for Responding to and Assessing Patient Assets, Risks, and Experiences)

Click the document icon above the assessment or the VIEW button to view a PDF in a separate browser.

The PDF will appear in a separate tab and can be viewed, printed, or downloaded.

The “View IDT Content” toggle filter defaults to show the IDT content.

To hide the content, click the toggle off.  The filter will remember the last setting.  Therefore, if the toggle is turned off, it will maintain that setting until turned back on.  

The reminder, “To Review Care Team Notes, use the 'View IDT Content' toggle in the Navigation Bar”, appears at the bottom of each care category to remind the user that documentation is hidden. 

IKC Hospitalization Alert

The Focus Patient Indicators banner appears in the Patient Status section of the patient's chart when they have an IKC Risk flag or have been hospitalized in the last thirty days.

The banner contains the core team and recent hospitalization, if available. The core team record displays the risk rating and reason. The recent hospitalization data shows the reason for admission, length of stay, admission and discharge dates, and primary and secondary diagnosis.

IKC Hospitalization Risk Rating

The Risk Rating (1-5) badge for IKC patients appears in the Patient Chart, Dialysis Schedule, Current Patients, and IKC Patients lists.

It will show before the IKC badge and does not differentiate between programs (i.e., CKCC/IKC).

The 1-5 ratings are documented in CWOW and based on the Core Team's analysis of whether or not the patient is at risk of hospitalization within the next 30 days. The data will come from the CWOW Clinical Management Tool (CCMT).

  • Rating 4-5: Team is concerned or very concerned
  • Rating 2-3: Team has some concern
  • Rating 1: Team has no concern

Hover over the badge, and the reason and the date of the last rating will be displayed.

The Current and IKC Patients list (under Patients) will also be able to filter by this data.
