Please note
To add, update, or remove any data in the patient's Profile, the patient’s DaVita care team must be contacted.
Patient Details
The patient's details, including MPI #, Facility, Race, Ethnicity, Phone, Address, and Emergency Contact information, will be displayed.
Pharmacy
Selecting the Pharmacy link will open a dialog allowing you to select a preferred pharmacy for the selected patient.
IKC Details
IKC (Integrated Kidney Care) program data will be displayed in this section, if the patient is associated with an IKC program. The IKC Program and contact information for the care team (RN, NP, Care Coordinator, IKCA) will be displayed.
Provider Details
The provider’s contact information for all facilities where the patient dialyzes will be displayed in this section.
Insurance Details
The patient’s primary, secondary, and tertiary insurance data will be shown here, if available.
Medications Eligibility
Selecting the Medications Eligibility link will open a dialog allowing you to view medication eligibility information for the selected patient.
