The Clinical Summary section of the patient chart will display information in the following categories: Prescriptions, Medication Orders, Treatment, COVID-19, Problems, Patient History, Allergies, Vaccinations, IDT Assessments, and Provider Notes. |
The data displayed is read-only. To add, update, or remove any of the data under this tab, the patient’s DaVita care team must be contacted.
Sections in the Clinical Summary
Prescriptions
The Prescriptions section includes e-prescriptions sent to external pharmacies for non-treatment medications. This list will include both prescriptions entered by providers using OneView and ones initiated by a care team member in CWOW and may also include Home medications.
Medication Orders
The Medication Orders tab contains treatment and home medications ordered for the patient in CWOW.
Treatment
Patient treatment data, such as modality, shift or access information, can be referenced in this section in order to provide quality care and make informed medical decisions.
COVID-19
The patient's COVID-19 history will be displayed here.
Problems
A list of documented Problems for the selected patient displays here with the ability to add, edit or delete them.
Patient History
This section displays the patient’s medical, surgical, social, and family history along with the date it was last updated.
Allergies
A list of the patient's documented allergies will be displayed along with the severity, the ability to add or edit the allergy, and the ability to sort the list by active or inactive.
Vaccinations
The patient’s history of vaccinations administered in a DaVita clinic within the last two years will show in this area.
IDT Assessments
This section contains all CWOW Interdisciplinary Team content and Monthly Summaries.
Provider Notes
A list of notes created for the selected patient will be listed here. You can edit, deactivate or view a PDF version.
