COVID-19 Attestation in OneView

This article will guide providers through the process of completing the COVID-19 vaccination attestation form in OneView.

Additional Dose & Booster Form

The COVID-19 Additional Dose & Booster Form is only available to providers who attested that they had received a COVID-19 vaccination.

Steps to Complete COVID-19 Attestation Vaccination Form

STEP 1: Go to oneviewdps.davita.com

STEP 2: Log in with your DaVita username and password

NOTE: Complete the attestation form once all vaccine doses have been administered or if you are refusing the vaccine(s).

STEP 3: Locate the COVID-19 Attestation Form in the My Activities & Reminders section of the Home Page

STEP 4: Click the COVID-19 Attestation Form to open

STEP 5: Select the radio button for the option that applies to you

NOTE: Additional information, based on the option selected, will be required.

STEP 6: Click the Acknowledge Here checkbox

STEP 7: Click Submit
