This article will guide providers through the process of completing the influenza vaccination attestation form in OneView or OneView Mobile.
Steps to Complete Flu Attestation in OneView
STEP 1: Go to oneviewdps.davita.com
STEP 2: Log in with your DaVita username and password
STEP 3: Locate the Flu Attestation Form in the My Activities & Reminders section of the Home Page
STEP 4: Click the influenza vaccination attestation message to open
STEP 5: Select the radio button for the option that applies to you
NOTE: If you’ve received a vaccination from an outside location, you will be required to indicate the location and date
STEP 6: Click the Acknowledge Here checkbox
STEP 7: Click Submit
Steps to Complete Flu Attestation on OneView Mobile
STEP 1: Tap the OneView Mobile app
STEP 2: Log in with your DaVita username and password
STEP 3: Locate the applicable influenza vaccination attestation message in the COMMs Hub Feed on the Home page or tap the Menu icon to select Comms Hub
STEP 4: Tap the Flu Form to open
STEP 5: Select the radio button for the option that applies to you
NOTE: If you’ve received a vaccination from an outside location, you will be required to indicate the location and date.
STEP 6: Tap the toggle to acknowledge
STEP 7: Tap Submit
