If a physician is incorrectly associated to a practice, they can disassociate from the practice at any time.
Steps to Disassociate the Physician Practice Association
STEP 1: Select Settings > Practice from the left navigation menu
STEP 2: Click the X under the My Practice section
STEP 3: Click the Confirm button
STEP 4: Enter the required search criteria to locate the correct practice
NOTE: If your practice is not listed, click Create Your Practice and enter the required practice information.
STEP 5: Click the Join button
STEP 6: Click the Confirm button to join the practice
