Clinical Details in the Patient Chart

The Patient Care > Clinical Details section of the patient's chart will display read-only comprehensive treatment and lab data relative to the patient’s modality.

Navigating Clinical Details

The Clinical Details section of the chart is organized by care category. 

The care categories can be collapsed or expanded for added flexibility.

An orange notification displays to the right of the collapsed care category header when a patient has lab results within the past 30 days that are considered out of range.

Navigate directly to a specific care category by selecting it from the Go To dropdown or scroll through the care categories using the scrollbar.

If a care category is selected from either the "Go To" dropdown or from within the encounter note, you will be taken to that care category section with the data automatically expanded.

Please note

The “Go To” dropdown will not be available if you use Internet Explorer (IE) 11 or older versions of the web browser. Chrome is the preferred web browser.

Care Categories

The Clinical Details section of the chart is organized by care category. Details about each care category are listed below.

Patient Status

The Patient Status section shows finalized patient status comments and encounter dates from the past 90 days. These comments can be copied into current encounter notes.

Fluid/BP

The Fluid-BP care category displays up to 120 days of treatment data, including target weight, treatment duration, and blood pressure.  Same-day treatment data is shown if the patient is receiving hemodialysis on the current day. Missed treatment and hospitalization events, and when the patient has not met their prescribed amount of treatment will also display here.

Adequacy  
Access The Access care category contains details regarding the patient’s dialysis access. Details include primary and secondary access as well as active versus inactive accesses.
Anemia  
Nutrition  
Mineral/Bone Disorder (MBD)  
Infection The Infection care category contains Microbiology, Serology and Body Fluid (for PD patients only) results. Up to two years of results may be displayed. 
Psychosocial/TX Modality The Psychosocial/TX Modality care category appears in the Clinical Details section of the patient's chart for CWOW patients only. This section contains information on transplant status as well.
Other

This care category has lab results that are not in any of the previous care categories. Tests ordered that are not in the normal monthly test orders appear first. The remainder of the results can be viewed by clicking "Show More". The list is in alphabetic order.

The  Other care category contains four CWOW care categories that don’t align with OneView care categories. They are Comorbidities, Health Maint./Safety, General, and Medication. 

Amount of Data Displayed

In some care categories, such as Fluid/BP, treatment data only displays if it is available for the patient's specific modality.  For instance, a PD clinical visit may only show Target Weight and BP Sitting, while an In-Center Hemodialysis patient may have Pre and Post values. Another example is when patients transition to PD from hemodialysis. Both parameters may be visible until the patients’ historical data is fully transitioned to PD. Similarly, a PD patient may have up to 70 lab results in the Adequacy care category, while an In-Center Hemodialysis patient may only have 12.

Lab Data

Up to 720 days of lab data can be retrieved in the Adequacy, Anemia, Nutrition, and MBD care categories.  

Please note

Initially, 180 days of the most common lab data will be displayed, with a "Show More" option to view the remaining data.  

Hovering over a lab value will reveal a tooltip with the result value, test code, reference range, collection date and time, and laboratory name.

Peritoneal Equilibration Test

For PD patients, the PET (Peritoneal Equilibration Test) results and date are displayed in a section named “PET Transport Classifications” below the Adequacy care category. 

The section will include 5 ratios along with their corresponding values.

The 5 ratios are 0HR D/P, 2HR D/P. 4HR D/P, 2HR D/DO, and 4HR D/DO.  Each ratio will fall into a specific range of Low, Average Low, Average, Average High, or High.  

Click the ratio data to view the associated value ranges for that ratio.

Prescribed Treatment Medications

Under the Anemia, Nutrition, and MBD care categories, the prescribed treatment medications that were active for that lab draw date will display.

An orange dot to the right of a medication dosage indicates that there are multiple active orders.

When you hover your cursor over the medication dosage, a tooltip will appear with the medication name, dose, unit, route, frequency, start date, discontinue date and reason, end date, and protocol, if available.

Please note

To add, update, or remove medication data, the patient’s DaVita care team must be contacted.

The "LABS" tab is the default view under the Anemia, MBD, and Nutrition care categories.  Hover over lab value to obtain additional information.

The "MEDS" view shows the actual date and dosage of a treatment medication given to a patient in a DaVita dialysis facility.

Study medications will be included but not any medications taken outside the treatment.

When you hover your cursor over the medication dosage, a tooltip will appear with the medication name, dose, unit, route, frequency, start date, discontinue date and reason, end date, and protocol, if available.

Please note

Treatment medications that are started and discontinued between 2 lab draw dates will not display in LABS view. Please use the MEDS view to see the delivered medications.

Clicking the Mircera informational tooltip will display details about the patient’s next dose of Mircera and an email address for questions or comments.

Treatment Details

Under the Adequacy, Nutrition, and MBD care categories, the following treatment details will show:  Tx Time, Last Delivered date which indicates the last treatment for which the listed treatment parameters were delivered, Dialysate Potassium, Calcium & Bicarb, Heparin Loading Dose and Heparin Hourly Dose, Heparin Concentration, and Prescribed BFR, Dialyzer for hemodialysis patients.

For CAPD or CCPD patients, Target Wt, Total Fill Vol 24, PD Type, Total Fills, Dry, and Tidal treatment parameters will display.  Click "SHOW MORE" to expand and see the full details. 

Care Activities and Patient Notes

The care activity and patient notes finalized in CWOW by the care team within the last 6 months are available in the Clinical Details section of the patient’s chart. 

They display within the notes carousel in the associated care category, along with the provider notes.

The care activity and patient notes may contain a title, completed date, comments or details, and care category tags.

DaVita-approved education materials provided to the patient are documented with the associated care team note.

The Mandatory Reporting of Abuse or Neglect assessment is only visible to providers and will have a Sensitive information indicator. 

Practice staff will not be able to view this assessment.

Barriers

Barrier pathway care activities provide valuable insights into the specific obstacles your patients may encounter in accessing care.  The DaVita care team will document these barriers to offer detailed insights and additional comments.

The barrier status appears for in-progress care activities and allows you to view barrier data related to access, placement or removal, psychosocial reasons, transportation, and key obstacles that affect patients' completion of necessary steps and activities required for their care.  The barriers are displayed in the Patient Status section of the Clinical Details tab.

 An orange dot indicates a New Barrier and  the green dot shows Barrier Resolved

Regardless of the status, the barrier name, the date created or updated, optional additional teammate comments, and the name of the teammate who created or updated the barrier will be visible.
