Encounter notes can be started from the Dialysis Schedule or the Patient Chart.

Starting a Note
Via the Dialysis Schedule: Click the modality-specific +C or +L button or the blue plus sign to view a full list of note types. The comprehensive note includes the specific modality type in the encounter name to ensure consistency in documentation. Selecting a note type will open a new note in the patient's chart.
Via the Patient Chart: Click the NOTE button to view a list of note options and start a note. The NOTE button is visible in all sections of the Patient Chart, and the note options are the same.
Encounter Note Types
After selecting to add a note, you will select the type of note. The note types have the same fields and basic functionality except for the following differences:

Encounter Date and Facility
The date the encounter occurred and Facility where the encounter was performed are required fields in the encounter note. The Date of Encounter will default to today's date when a new note is created. The date can be modified. You can backdate up to 180 days from today, but no future dates are available for selection.
Click the calendar to select a date prior to today.
Click the Facility dropdown to change the facility. |
Encounter Completed With Telehealth
The “Encounter Completed With Telehealth” statement allows a provider to document that the encounter was completed with telehealth. |
The option is available in all note types except Transition of Care and Non Visit Note.
For home encounter types (PD, HHD, and Home Training), a blue Telehealth hyperlink is available. |
When clicked, the Telehealth link will take you directly to DaVita Care Connect (DCC) to allow you to complete a telehealth visit. DCC will open in a new browser with you already logged in.
