Patient Details drawer is opened as the default. The information is read-only. Tapping the blue telephone number will prompt you to dial the phone number listed. |
Patient Information Banner
The patient information banner is located at the top of the screen and will contain the patient’s name, age, date of birth, gender, and dialysis facility. Badges may display and can be clicked to give you additional details.
Badges
For patients participating in an IKC (Integrated Kidney Care) program, the “IKC” badge will display in the patient banner. |

Tap the badge, and a drawer opens with the IKC Program name, IKC Care Team member names and role (Nurse Practitioner, IKC Nurse, Care Coordinator, IKC Assistant, Social Worker), email addresses, and patient’s nephrologist, if available. | |
The Recent Hospitalization (Recent Hx) badge alerts you of the patient’s hospitalization status. The badge displays if the patient has an admission date within the last 30 days. The badge will continue to show for 30 days after the patient is discharged. | ![]() |
Click to view the details of the most recent hospitalization entered into CWOW. Only the most recent hospitalization shows if there have been two events in the last 30 days. If the admit and discharge dates differ, the number of days the patient was hospitalized appears by the discharge date. | ![]() |
The Recent ER badge alerts you that a patient has been in the hospital for less than two midnights. |

A target weight badge displays for patients who have not met their target weight. If the patient's post-treatment weight is either above or below the target weight by 1 kg or more in 2 out of the 3 most recent treatments, the indicator appears. | |
The “COVID Vaccinated” badge is displayed on the patient banner if a COV19-19 vaccination record was entered. Tap the badge, and the drawer opens with the date(s) that the vaccine(s) were given. The date(s) display the most recent to oldest date. The COVID-19 vaccine status does not show for patients that have refused the vaccine. |
To return to the Patient Chart options, tap or slide the blue drawer navigation bar located at the bottom of the screen. “Patients” located at the top right takes you to the patient search screen.
Providers
Providers will have the patient’s provider and contact information, if available. Tapping the blue telephone number will prompt you to dial the phone number listed. |
Insurance
Insurance will have the patient’s insurance information, if available. |
