The Fluid/BP care category displays a detailed view of clinical data supporting patient care and encounter documentation. |
Treatment Data and Parameters
The Fluid/BP section includes information such as Target Weight, BP, Home Treatment Records, treatment times (and indicator if it is not met), and missed treatments and hospitalization events.
In some care categories, such as Fluid/BP, treatment data only displays if it is available for the specific modality. For instance, a PD clinical visit may only show Target Weight and BP Sitting, while an In-Center Hemodialysis patient may have Pre and Post values. Another example is when patients transition to PD from hemodialysis. Both parameters may be visible until the patients’ historical data is fully transitioned to PD. Similarly, a PD patient may have up to 70 lab results in the Adequacy care category, while an In-Center Hemodialysis patient may only have 12.
Home Treatment Records
Home treatment record (HTR) data entered into DaVita Care Connect (DCC) via a patient-facing app for new peritoneal dialysis (PD) patients will be displayed in the Fluid/BP section.
The “Patient Home Treatment Records” option will only be available if home treatment records are entered into DCC within 120 days from today's date. |
Clicking on the “Patient Home Treatment Records” will expand or collapse the section. The data can be shown in a table or graph view when the section is expanded.
A disclaimer will display to the right of the view selections if the data has been manually entered by the patient. |
From the table view, the Dextrose % icons are color-coded to represent the type of PD bags used in the treatment.
Prescribed Time and Target Weight
In the Fluid/BP section, the patient’s current Prescribed Time and Target Weight are available in the current date column. Hover your cursor over the data, and additional information will appear. |
|
A target weight indicator appears if the patient's post-treatment weight is above or below by at least 1 kg in two of the three most recent treatments. |
|
Hover over the information icon next to the Post Wt field for more details. |
Prescribed Treatment Time Not Met
An indicator displays for treatments where the prescribed time was not met. When the patient’s treatment time is less than the prescribed time by 10% or more, the "Actual TX Time" appears in orange.
Hover over the time and a tooltip displays the time cut along with the percentage of the prescribed time completed in parenthesis, the actual treatment time, date, and clinic. |
The percentage of treatment time compared with prescribed time is calculated whether or not a care team note is entered.
Missed Treatments and Hospitalizations
Missed treatments and Hospitalizations are published as a column in the Fluid/BP care category. The MTx indicator designates a missed treatment and HOSP signifies a hospitalization. A count will show before the MTx or HOSP if consecutive days are missed.
When you hover your cursor over the MTx indicator badge, a tooltip appears that contains the missed treatment date and reason. |
Hover over the HOSP indicator badge and a tooltip appears that contains the dates, reason, details, and diagnosis.
Hover over the HOSP indicator badge and a tooltip appears that contains the dates, reason, details, and diagnosis. |
Missed treatment, hospitalization, and recent ER visit care team notes are not always supplied and therefore may not display with a reason. We only provide what is documented by your care team.
Recent ER Visit
The ER indicator designates the patient spent less than 48 hours in the hospital. |
|
When you hover your cursor over the ER indicator badge, a tooltip appears that contains the admit date and discharge date, admit reason and discharge diagnosis, if available. |
Treating Elsewhere
If the patient is treating outside of a DaVita facility, the TxEw (Treating Elsewhere) badge appears in the date row of Fluid/BP, as well as Anemia, Nutrition, and MBD care categories under the "MEDS" tab. |
|
Hover your cursor over the gray TxEw badge, and a tooltip appears that contains the started on and ended on dates, reason, home training elsewhere, and data source. |
