Rounding Without Network Connectivity

When OneView is unavailable due to a network or system outage, there are alternative options for documentation while rounding.

The Provider Dialysis Rounding Note template is a blank note template that you can print in bulk and give to your physicians when they arrive to round on patients. Your facility teams can print reports in CWOW that summarize patient data for Physicians using the IDT Patient Profile Worksheet.

Rounding on Paper for Providers

  1. Notify the facility that you will be rounding in their facility
    1. Request they print the IDT Patient Profile Worksheet for all patients for the day

NOTE: The IDT Patient Profile Worksheet is a single patient view of the most relevant information needed during rounding.
  1. Indicate which section should be omitted, if needed

  1. Collect the rounding worksheet from the facility when you arrive
  1. Conduct rounds with the worksheet
  1. Return your worksheet to the facility after rounds

Rounding on Paper for Teammates

The IDT can be proactive during system downtime by printing the IDT Patient Profile Worksheet for all providers rounding that day. To print the worksheet, follow the steps below.

  1. In CWOW, navigate to Reporting
  1. Select the IDT Patient Profile Worksheet
  1. Select Patients and click Next

NOTE: The worksheets can be filtered by Primary Nephrologist.
  1. Select the appropriate sections
  1. Deselect Medication(s) Eligible for DaVita to Provide
  1. Click the Print Report button
  1. Give the rounding worksheet to the provider when they arrive
  1. Collect the worksheets after the provider has conducted their rounds

Patient Roster Report

To see a list of patients you should plan to round on, please connect with your care teammates who can provide a Patient Roster Report by following these steps:

  1. In CWOW, navigate to Reporting
  1. Select Patient Roster: Facility Census List
  1. Click Filter: Primary Nephrologist (and any other filters that could be helpful)  
  1. Select all patients from the filtered list, click Next
  1. Select include “Primary Nephrologist” and “Treatment Day/Shift” 
  1. Click Print Report button

Rounding Tracker Template

To track which patients have been seen in a month and which encounters have been completed while OneView is offline, please continue to manually document on paper for the time being.  

  • You and your care team can use this Rounding Tracker Template.  Feel free to use a different layout or adjust for your clinic’s purposes.
Please note

Ensure copies of your rounding notes are left in your center for your care team.
