Previously, access to these charts was only available through the Dialysis Schedule. When using the Search field, multiple records will display for the patient with the difference being the facility. Either option will take you to the same chart; patients only have one chart in OneView.

In addition, all facilities where the patient dialyzes will now be listed under the Profile section of the patient’s chart.

NOTE: When viewing a chart for a patient being seen in multiple facilities, the facility that shows in the patient banner (at the top of the chart) is the facility where the patient’s vitals were last documented.

Former Patients

The ability to view a list of patients no longer in your care is now available from the Search field or by going to Patients > Former Patients. From the Search field, after typing the name of the patient you are looking for, the option to view the list of Former Patients will be available at the bottom of the results list. When viewing Former Patients from the Patients menu, a list of patients deactivated within the last 90 days will be displayed by default to assist with finding recent patients easily.

Charts for patients that have been out of a physician’s care for more than 90 days will not be accessible in OneView. In order to view these charts, call Physician Support to request access.

New encounter notes can only be created for former patients if they have been in your care anytime in the last 90 days. If the patient has not been in your care for 90 days or more, new encounter notes cannot be created in OneView. It is possible to create an encounter note for a former patient that has been in your care in the last 90 days; however, you will not be able to set a date for an encounter more recently than 14 days from the last documented encounter.

HHD Order Justification – Electronic Form No Longer Available

As of February 28, 2021, the ability to submit electronic forms for HHD Order Justification (or More Frequent Dialysis) will no longer be available in OneView. You should return to using the paper version of the form until this feature is re-released at a later time.

(MOBILE ONLY) Access to the Dialysis Schedule

The ability to view a list of your patients being seen in a DaVita facility within the next 7 days is now available in OneView Mobile. Your Dialysis Schedule will be available through the Menu option with the ability to filter based on facility, day of the week, shift and COVID-19 status. When viewing your list of patients, you will see their name, age, date of birth, gender, shift and modality. You will also have a visual indication whether an encounter note has been created for a particular patient for the current month with details on the type of encounter it was. A filled green box indicates the encounter note was finalized while a dotted yellow outline represents an in-progress encounter note. A white box indicates no notes were created for that type of encounter for the current month. Tapping a finalized encounter note (filled green box) will display a read-only version of the encounter note.

If location services are enabled and OneView finds a facility within a radius of the user (100 meters/.06 miles), the patient list for the DaVita dialysis facility nearest to their geo-location will auto-populate and won’t need to be manually selected. The option to modify the location is available through Filter.

NOTE: This feature is only being released to a pilot group at this time.

(MOBILE ONLY) Patient Chart Redesign

The patient chart redesign utilizing a “drawer” style of navigation common in mobile devices that allows you to view additional data by swiping the drawers up and down (open/close) has been released to all mobile users at this time. To view more information on this redesign, click here.

Encounter Note Updates

New Note Type – Home Training

A new type of encounter note to document when training on the Home modality was performed is now available when you click “Start Note” from within the patient’s chart. Home Training encounter notes will not be included in visit totals in the Monthly Visit Summary report.

Amendment Workflow Update

In the Clinical Summary section of the patient’s chart, the ability to amend a finalized encounter note is now available for any provider. Previously, only the provider who finalized the encounter note was able to make an amendment. Documentation as to who made the amendment will display in the PDF version of the encounter note. Also, after 90 days has passed, the option to amend the note will no longer be available.

Backdating Restricted to 90 Days

The ability to backdate an encounter note has been limited to 90 days. When selecting the date of the note, dates prior to 90 days will now be greyed out and not available to select.

Rules for Modifying Notes > 90 Days Old

You will no longer be able to amend, edit, or finalize encounter notes that are older than 90 days. The option to reactivate or deactivate the encounter note will still be available.

Practice Updates

Ability to Edit Practice Address

Once you have confirmed your practice association within OneView, if you notice your practice address is incorrect you will now have the ability to edit your information by navigating to Settings > Practice and selecting the edit icon. Modifying the practice address will update the address for all providers associated with that practice. Editing a practice address can only be completed by a physician.

Access Termination for Non-Provider Users

The ability to remove a practice association for a non-provider staff member is now available through Settings > Practice. This action can only be completed by a provider, and will terminate the non-provider staff member’s access to OneView. The non-provider staff member will receive a confirmation email notifying them of the change after the termination is completed.

Dialysis Schedule: Filter by Modality

In order to simplify viewing a list of your patients by modality, we have added the option to filter the Dialysis Schedule by ICHD or HHD, PD. If you are viewing a PD or HHD patient and click the plus sign to start an encounter, the respective encounter note type will open automatically. The ability to view patients by modiality is only available for Snappy patients at this time.

Patient Chart: ESRD/AKI Mgmt Updates

HIFs Graph – SHAPE Roxadustat

The SHAPE 10 graph located under the Anemia care category of the patient’s chart has been renamed to “SHAPE TRENDS” and will now include data on the Roxadustat protocol as well as the SHAPE 10 protocol. Roxadustat data will display in a teal color while SHAPE 10 will continue to display in blue. The date in which the Roxadustat medication was started will be displayed with a vertical line. For Roxadustat, a white background on the graph indicates the patient is participating in a clinical study for the medication. If the graph background is light teal, this indicates the patient is on the Roxadustat protocol.

Care Category Alerts – Out of Range Labs

When a patient has lab results within the past 30 days that are considered out of range, an orange notification will display to the right of the care category header indicating the provider should review this information.

(CWOW Only) Timestamp on Lab Details

For CWOW patients only, when you hover over a lab result in the ESRD/AKI Mgmt section of the patient’s chart, the time displayed will be determined by the timezone set through the browser. This will allow the user to view the time the lab was drawn in their own timezone.

MVS Report: Column Added for Patient ID

The Monthly Visit Summary Report (available through the Reports menu) has been updated to include a column for the patient’s ID.

(MOBILE iOS ONLY) Push Notifications for Unreviewed Orders

If notification settings are enabled through your phone settings, OneView will now push a notification every 7 days if there are one or more unreviewed orders available for the provider.

Release 1.1 (January 21, 2021)

COVID-19 Vaccine Status in Patient Banner

The patient’s COVID-19 vaccination status will be displayed in the banner at the top of the patient chart if vaccination records have been entered into Snappy. When you hover over the status, a dialog will appear with the date(s) of the vaccination. This information will also be displayed in the Clinical Summary section of the patient’s chart under Vaccines.

COVID-19 Vaccination Attestation Form in Comms Hub

Providers will be able to document their COVID-19 vaccination statuses by completing a form similar to the Flu attestation through COMMS Hub. The ability to document whether you, as a provider, received or declined the vaccine is available. If you select the option that you received the vaccine, you will be asked to document the manufacturer and the date the vaccine was administered.

Encounters: Finalize Button Always Available

The requirement to view all data present in the ESRD / AKI Mgmt section of the patient’s chart prior to the Finalize button being available in an encounter note has been removed.

Patient Chart: Section Accordions and Go To Links

The ability to minimize and expand each care category in the ESRD / AKI Mgmt section of the patient’s chart is now available. Release 11.1 (December 3, 2020)

Encounter: H&P Indicator

The option to document whether a History and Physical (H&P) was completed during an office visit is now available through all encounter note types. The “Completed H&P” option is located at the top of the encounter note with the option to select “Yes” if it was completed. Designating the completion of an H&P can only be done once the History section of the encounter note (located below the “Completed H&P” option) has been reviewed. If “Completed H&P” is set to Yes, an indicator will display next to the encounter note in the Clinical Summary section of the patient’s chart in order to make it easier to locate those notes.

Patient History

All encounter note types have been updated to include free-text fields to document patient information related to Medical, Surgical, Social and Family History. Comments documented in a previously finalized Falcon Silver or OneView encounter note for each field will display in subsequent OneView encounter notes with the option to edit the comments, if necessary. A new section displaying documented patient history has also been added to the patient’s Clinical Summary for easy reference.

Visiting Patients

Patients being treated at a facility other than their “home” location will now display on the Dialysis Schedule.

Patient Vitals Added to Encounter PDF

When the patient’s Fluid or BP information is documented in an encounter note, the information will now display on the PDF version of the encounter note (available through the Clinical Summary section of the chart). The PDF will include data from the last 3 times Fluid and BP information was documented. Incomplete data will be represented by an asterisk.

Labs Not for ESRD Treatment Comments

Comments related to Labs Not for ESRD Treatment documented through the queue or within an encounter note will now display in the ESRD / AKI Mgmt section of the patient’s chart under Infection in a carousel. The comments will also display in the PDF version of a finalized encounter (available through the Clinical Summary section of the chart). The date the comment was made, as well as the lab, and the specific comment documented will be displayed in both the carousel and the PDF. When viewing the carousel in the ESRD / AKI Mgmt section of the chart, a “test tube” icon will indicate the comment was documented through the queue, while a “document” icon will indicate the comment was documented in an encounter note. Clicking a comment originating from the encounter note will open a PDF of the encounter note.

Labs Not for ESRD Treatment documented through CWOW will now display in the OneView encounter note as well. In a future release, this information will also be displayed in the queue.

Practice Associations

The ability to associate yourself and staff members with a practice has been added to the application. Under Settings > Practice, providers will have the ability to confirm the practice affiliation we have on record. If the defaulted information is not accurate, the ability to associate with a different practice will be available. Providers and staff members assigned to your practice who have not confirmed their association will show as pending. Providers will have the ability to add Practice Staff members which will invite that person to login to OneView. Release 9.1 (October 8, 2020)

Dialysis Schedule

New Buttons for Starting an Encounter

The Dialysis Schedule has been redesigned to display relevant encounter information prominently so a provider can plan their next actions appropriately. There is now a column (“Comp”) that will display the most recent Comprehensive note from the current calendar month and a column (“Additional MCP Notes”) displaying up to 3 additional MCP notes. For AKI patients, the Comp column will be blank and the Additional MCP Notes will be labeled “AKI”. If an encounter was created in the current calendar month, the date of the encounter will display on the bubble, otherwise a “+C” or “+L” (allowing you to create a new one) will be present. If a bubble has a date listed and an orange dashed outline, this indicates the encounter is still in progress. If the bubble has a date and a green-filled background, this indicates the encounter has been finalized. Clicking on a bubble with a date will open that encounter. Clicking anywhere else on the row will open the patient’s chart. Clicking the blue plus sign to the right of the bubbles will display the option to create a new Comprehensive or Limited encounter note. For AKI patients, the blue plus sign will display the option to start an AKI encounter.

Workflow Change – Starting/Closing an Encounter

Closing an encounter note that was started from the Dialysis Schedule will now take you back to the Dialysis Schedule, as long as you haven’t navigated elsewhere within the chart in between opening and closing the encounter.

Manually Entered COVID-19 Results

COVID-19 results manually entered in Snappy will now display in the ESRD/AKI Mgmt section of the patient’s chart in the Infection section.

Non-ESRD Labs in the Encounter – CWOW Data

Non-ESRD lab results that were entered through CWOW in the last 90 days will now display in the encounter in OneView. This data will not be available in the Non-ESRD Labs queue until a later release.

Group Agreements will now be available under Invoice & Attestations in OneView Mobile, if applicable.

Release 8.2 (August 27, 2020)

ESRD/AKI Mgmt - Shape 10 Graph

A graph displaying Hemoglobin (Hgb) levels and Epogen doses for patients that are on the new Shape 10 anemia management protocol has been added to the Anemia care category of the ESRD/AKI Mgmt section of the patient’s chart. The graph also includes a suggested dose (coming from the third party vendor), the prescribed dose of Epogen, and missed treatments. By default, the graph will be collapsed but can be expanded by clicking “+SHAPE 10 TRENDS”. Upon opening the graph, 180 days of data will load with the ability to scroll to the right to view additional data. The date in which the Shape 10 protocol was started will be displayed with a vertical line. A blue background on the graph indicates the patient is on the Shape 10 protocol. A white background indicates a time prior to the protocol starting, or after the protocol was discontinued. If the patient is within their first 3 months on the Shape 10 protocol, a message will display indicating they are in this initial transition period.

Encounter Note Updates

Non-ESRD Labs (Snappy Only)

The ability to document labs designated as not for the treatment of ESRD is now available through the encounter note for labs (ordered within the last 90 days) classified under Nutrition, Infection or Other. This functionality is available in the Comprehensive, PD and HHD encounter note types for Snappy patients. Physicians may select either “Changed Care Plan” or “No Change” and will also have the ability to document comments, if necessary. A “Past Due” indicator will display if the lab has not been reviewed within 30 days of the resulted date. Entering documentation for a non-ESRD lab through the encounter note will remove that lab from the Labs Not For ESRD Treatment queue and vice versa.

Save Button Removed

The SAVE button has been removed from all encounter note types due to the fact that it is no longer necessary. OneView performs an auto-save function every few seconds. The time in which the encounter note was last saved is displayed where the SAVE button used to exist.

Collapsible Sections (Limited Encounter)

When you start a Limited encounter note, the Overall Patient Status and Fluid/BP sections will be expanded and all other sections will be collapsed. For all other encounter note types, all sections start expanded. To collapse a section, click the dash located to the left of the section title. To expand a section, click the plus.

ESRD/AKI Mgmt Updates

PET Profile (CWOW)

This data previously displayed in the Adequacy table but was segmented for ease of use. When the “PET PROFILE RESULTS” link is clicked, a new dialog will display with the applicable information.

Monthly Lab Tests (Snappy and CWOW)

Certain labs that are ordered on a regular basis have been moved to a new section labeled “Monthly Lab Tests” under the Infection care category. The labs that will display in this new section were previously displayed under the Other care category. This change was made to improve the organization of lab results.

Generating Reports

The Reports module has been redesigned as a two-step process to generate a report. The new workflow will allow you to download reports generated at a previous time, and will include the date it was created. After selecting your report criteria and clicking “Create Report” a message notifying you the report is being processed will display. Once the report has been created, the option to “Download Report” (with the “last created” date and time) will display in the report area. This new workflow is applicable to both the Monthly Visit Summary and All Patient Insurance reports.

(MOBILE ONLY) Labs Not for ESRD Treatment

The ability to sign or dispute orders designated as “not for the treatment of ESRD” is now available on Mobile. A task card will display on the Home screen if there are outstanding orders. Tapping the task card will take you to the queue where you will select “Reviewed, Changed Care Plan” or “Reviewed, No Change” prior to submitting the documentation. A reason will also be required if you note that the care plan changed. Once you select a “reviewed” designation and click “SIGN AND SUBMIT”, that order will be removed from the queue. This new feature mirrors the functionality released on the web app in the 2020.6.1 release.

Release 8.1 (August 6, 2020)

Release 7.1 (July 9, 2020)

Enhancements to Snippets

Several modifications were made to the Snippet functionality to improve the user experience. Snippets created in a Comprehensive encounter will now be available in a Limited encounter and vice versa, eliminating the need to save them twice. Snippets can now be used in the Access comment box whereas they were not available in this field previously. The ability to add a Snippet (in any field) was added to the workflow of amending a note, which is completed through the Clinical Summary section of the patient chart. Finally, in order to make it clear that a Snippet was saved, we added a success message which will appear above the comment field after a Snippet is saved so you know the action took place.

Telehealth Link to DCC (HHD Notes)

The Telehealth link within the HHD encounter will now take you directly to DaVita Care Connect (DCC) to allow you to complete a telehealth visit. DCC will open in a new browser with you already logged in.

Clicking the “Yes” pill button, located at the top of the note, will designate the encounter as a telehealth encounter, and will show as such on the Monthly Visit Summary report and the PDF version of the encounter.

Snippets

The ability to create and select from a pre-defined list of common statements has been added to all encounter note types. Snippets will be unique to each user, encounter type and field within the encounter note. This means, for example, if a Snippet was created in the Overall Patient Status field of a Comprehensive note, that Snippet will not display in the Anemia comment field of a Limited note. The only exceptions to these rules are: Transplant Status – Snippets will be available across all encounter note types for an individual user; Access – Snippets will not be available yet. If you used Snippets in Falcon Silver, those Snippets will be available in OneView.

Adding a previously saved Snippet to an encounter field is done by clicking the blue comment bubble with quotes located to the right of the text box field. Saving a new Snippet is done by clicking the blue comment bubble with a plus sign located inside the text box field. Patient Chart Updates

Copy Care Category Comments

Care category comments entered in a previous, finalized encounter created in OneView will now be available to pull into and edit in a new encounter. Previous comments will display beneath their respective care category in the ESRD/AKI Mgmt section of the chart. Clicking the document icon next to the comment date (in the ESRD/AKI Mgmt section of the chart) will generate a PDF of the encounter the comment was documented in.

New Care Category - Access

A new section for Access information has been added to the ESRD / AKI Mgmt section of the patient’s chart. The ability to filter based on the status of the Access (active/inactive) is available. A section to document findings related to Access has also been added to the encounter note. Access information is coming from Snappy.

You will be able to filter the list by All Patients, My Patients or Covering Patients, as well as by Facility or Modality. If a provider’s name is present under the Facility name (on the right side of the list), this indicates the OneView user is not the primary nephrologist for that patient.

Release 3.1 (March 12, 2020)

ESRD / AKI Mgmt Updates

Several additions have been made to the data displaying under the ESRD / AKI Mgmt section of the patient’s chart for Snappy patients. Details are below.

Same Day Treatment Vitals

When looking at the Fluid/BP section of the chart, if the patient is currently receiving a dialysis treatment, their treatment vitals will display in the first column. An orange dot indicates there are multiple results for that vital. When you hover your cursor over the vital, a tooltip will appear with the vital data and treatment date.

Treatment Medications – MDB and Nutrition

You will now see details on prescribed and actual treatment medications applicable to Mineral/Bone Disorder and Nutrition by toggling between the two views – Lab Dates and Treatment Dates. The prescribed Snappy or Med Check home and treatment medications that were active for a particular lab draw date will display by default under the “LABS DATES” tab. To view the actual dosage of a treatment medication given to a patient while in a DaVita dialysis facility for a treatment date, select the “TX DATES” tab. Study medications will be included, however, medications taken outside of the treatment will not be included. A “P” icon represents a protocol medication, while a “P” with a slash indicates the medication is non-protocol.

Patient Insurance

In the Profile section of the patient’s chart for both Snappy and CWOW patients, you will now see the patient’s current insurance information.  The primary, secondary, and tertiary insurance will display if available in Reggie.  The insurance name, address, phone number, effective date, group number, insurance ID, and coverage start date are listed, if present.

Encounter Note Updates

Several additions have been made to the encounter note. Details are below.

Transplant Status

A section has been added to all encounter note types for document information related to transplant status. The provider can document if the patient is a transplant candidate, comments related to the status, the patient’s transplant status, and status date.  There are no required fields within the section. If transplant information is documented in OneView, the most recent information will display in the PDF version of the encounter note. The most recent transplant status will not appear on the PDF for Falcon Silver encounter notes.

Amend Finalized Confirmation

In the Clinical Summary section of the patient’s chart, a pop-up asking to confirm your action will appear if the “Amend this Note” icon is clicked for a finalize encounter note. The pop-up will ask to verify the action of creating an amendment to the finalized note.  If “Yes” is selected, the note will open so the amendment can be documented.  When “Cancel” is selected, it will return you to the Clinical Summary screen.

(MOBILE ONLY) Patient Search

The Patients menu was removed from the left navigation. To search for a patient, use the search bar at the top of the home screen.

(MOBILE ONLY) Patient Chart Redesign

The patient chart was redesigned to utilize a “drawer” style of navigation common in mobile devices that allows you to view additional data by swiping the drawers up and down (open/close). The chart will be organized into 3 sections of read-only information for Profile, Clinical Summary, and ESRD/AKI Mgmt information. To add, update, or remove any of the data under any area of the patient’s chart, the patient’s DaVita care team must be contacted.

To view more information on this redesign, click here.

Care Category comments are visible but cannot be acted upon.

To view a PDF of the encounter in which the note was created, click the document icon next to the comment date and the PDF will open in a separate browser.

Read-Only Medications List

A list of the patient’s current and historical medications will now be accessible from the ESRD/AKI Mgmt section of the patient’s chart for added convenience. This list is the same list you will see under the Clinical Summary section of the patient’s chart. The list will display all active medications by default. Selecting the “ALL” option will display the patient’s inactive medications along with the active meds. In the ALL list, inactive medications will be grey and italicized and active meds will be bolded.

Patient Chart: Lab Section Removed

The Labs section of the patient’s chart has been removed for both Snappy and CWOW patients.

For Snappy patients, the data will now live under the ESRD/AKI Mgmt section of the chart under the respective Care Category. For instance, you will find Creatinine results under the Adequacy section and Hemoglobin results under Anemia.

No lab results will display for CWOW patients at this time.

Labs Displayed in PDF Note

For Snappy patients, a subset list of laboratory results will now display in the applicable care category in the encounter note allowing the user to comment on specific lab results. The most recent lab value and draw date from the previous 6 months will display. The same list of labs will also display in the PDF version of the note, if the lab was documented in the encounter note. The list of applicable labs includes: spKdt/V, Hemoglobin, Iron Saturation, Ferritin, Albumin, Potassium, Phosphorus, Corrected Calcium, and PTH-Intact.

(MOBILE ONLY) Lab Notifications from DaVita Labs

In order to provide more visibility into urgent lab results coming from DaVita Labs, we will now display notifications for these results, as a feed card, on the home screen of OneView Mobile. This feature will provide visibility of labs to the physicians and care teams to aid in making timely decisions. The lab notifications displayed are based on DaVita Lab’s Urgent Notification policy and will be presented as a read-only card with the ability to view details when tapped, and will not include push notifications. These cards will exist on the feed for 12 months and will be displayed from newest to oldest. Tapping the card will display summary details about the patient and the lab result. The details screen will provide the physician and care team more data around that specific lab.

Release 10.1 (October 24, 2019)

Missed Treatment Indicator

In the patient’s chart under ESRD/AKI Mgt, if the patient has missed a treatment, a “MT” badge will display in the date row in the Fluid/BP section for the date that was missed. If the patient has missed sequential days of dialysis treatments, the number of missed treatments will display on the badge as well. If the patient missed a dialysis treatment on the same day as an office visit, the “MT” badge will display adjacent to the date. Hovering over the badge will give you the date and reason (if available) for the missed treatment(s).

PD Patients

OneView will now include patients receiving Peritoneal Dialysis, therefore PD-specific lab data may show in the ESRD/AKI Mgt tab under the Adequacy, Mineral Bone Density and/or Nutrition sections.

Protocol Orders

Protocol orders for patients in CWOW clinics will now be available in OneView for review. Signing the protocol order will serve as a signature for per-protocol orders, unless a provider has requested a change be made to a per-protocol order. When a change is made to a per-protocol order, a CHG order will result and need additional signature.

Treatment Tab - Fluid/BP

Fluid/BP data for up to 120 days of treatment will be displayed for HD patients in the Treatment section of the patient's chart.

Release 5.1 (May 17, 2019)

Patient Orders Removed

Previously, a CWOW patient's chart included information from the current diet, lab, and treatment orders, regardless of order status (signed or unsigned).  Due to the CWOW release R6 including an architectural change to orders, this functionality will not work, and thus the Patient Orders section has been removed from the Clinical Summary tab of the Patient Chart.

New CWOW Order Types

There are three new types of Treatment Orders - Additional Day of Dialysis, Sequential Order, and Backup Hemodialysis Order.  An Additional Day of Dialysis Order is for those patients who need more than three treatments per week.  A Sequential Order is for an additional treatment to remove fluid (but not to perform dialysis).  A Backup Hemodialysis Order is ordered for in-center dialysis for a Home Patient.

Release 4.1 (April 18, 2019)

Users can search for their patients (and for those for whom they are covering) by name.  When the results list appears, clicking on a patient's name takes the user to the Patient Chart.

Patient Tabs

Up to four patient charts can now be opened simultaneously.  Navigation between charts is performed by clicking on the tab associated with the patient's chart.

Patient Chart

Users can open a patient's chart through the Patient Search feature or by clicking on the patient in the Dialysis Schedule.  The Patient Chart shows data from Snappy including Lab Results, Microbiology results, medications, and allergies (medications may also be shown from MedCheck).
