All Patient Insurance


The All Patient Insurance report includes a list of all patients’ details and the primary, secondary, and tertiary insurance data.


NOTE: It is your responsibility to ensure the accuracy of the insurance data included in the report before utilizing the information.

Providers can create the report by clicking the Create Report button.  Once the report is complete, the option to “Download Report” (with the “last created” date and time) will display in the report area.  The report will be generated in an Excel format

Non-provider practice users who join the practice can create and download the Monthly Visit Summary, All Patient Insurance and Patient Population List reports.  Unlike physicians, however, non-provider practice users must select the “Physicians” criteria for the report.  One or more physicians can be chosen for the report.

NOTE: Only physicians who have confirmed their practice association in Settings >Practice are listed in the dropdown.  Physicians who have not completed this process will not appear as a selection under the “Physicians” dropdown.  Please refer to the “Physician Practice Association” article for full details.

The information displayed in the report is detailed below.

Column Description
Patient Details The patient’s name, DOB, gender, address, FDODE (first date of dialysis ever), patient phone number, MPI, primary diagnosis and provider
Primary Insurance The primary insurance name, address, phone number, effective date, group number, insurance id, and coverage start date
Secondary Insurance) The secondary insurance name, address, phone number, effective date, group number, insurance id, and coverage start date
Tertiary Insurance The tertiary insurance name, address, phone number, effective date, group number, insurance id, and coverage start date
