Schedule
The Patients menu will provide the ability to populate the dialysis schedule, navigate to the patient charts, document encounters, view the time left on dialysis and access a list of all patients.
Dialysis Schedule
The dialysis facility nearby can be found and loaded if Location Services are enabled for your browser. Geolocation or Location Services uses Bluetooth and Wi-Fi hotspot locations to determine your approximate location. A browser pop up will appear if Location Services has not been setup. To enable, either allow within the pop-up or modify your browser settings.
The dialysis facility can be manually selected in order to populate the schedule if the location services are not enabled.
After the dialysis schedule is populated, the information can be filtered by the selections below.
Filter | Description |
Facility | The facility where the patients are receiving treatment. The options are the facilities where the provider is credentialed. |
Patients | The provider’s relationship with the patient. The default is My Patients. The options for a physician are: My Patients, All Current Patients, and Covering Patients. The options for an NPP are: My Patients, All Current Patients, Covering Patients, and each physician that the NPP extends for at the currently selected facility. The default is My Patients. |
Modality | The patient’s treatment modality. The default is ICHD. The options are: ICHD and HHD, PD. The ICHD filter option includes (In-Center Hemodialysis) and AKI (Acute Kidney Injury) patients. The HHD, PD filter option includes HHD (Home Hemodialysis) and PD (Peritoneal Dialysis) patients. Currently, this filter is only available for Snappy patients. |
Day | The patient’s day of dialysis. The default is today’s day. The options are: Sunday through Saturday. |
Shift | The patient’s shift. The default is All Shifts. The options are the dialysis facility’s shifts of operation. |
Station | For CWOW patients only, the station number is visible for patients receiving treatment. The CWOW patient’s assigned shift displays if the patient is not currently receiving treatment. Sort the column by using the arrows next to the column name. |
COVID-19 Status | COVID-19 status of the patient. The default is NO COVID-19 Filters The options are: No COVID-19 Filters, All COVID-19 Status, COVID-19 Positive, COVID-19 PUI Symptomatic, and COVID-19 PUI Asymptomatic. |
The Dialysis Schedule can be filtered by modality type for Snappy patients only. If the ICHD filter option is selected, the list includes ICHD (In-Center Hemodialysis) and AKI (Acute Kidney Injury) patients. The encounter note types available are Comprehensive, Limited and AKI (for AKI patients). If the encounter note has not been created for the current month, the indicator is blank (no date) and has a +C, +L, or +AKI available to add the encounter note.
NOTE: Physicians can edit and finalize in progress encounter notes started by a non-physician provider (NPP). A physician is not allowed to edit or finalize another physician’s note, and an NPP can not edit or finalize a physician or another NPP’s note.
The ICHD and AKI patient information displayed is detailed below.
Column | Description |
Patient Name | The name of the patient. The information will be displayed as last name, first name, and will include the patient’s COVID-19 status, IKC badge, age, DOB, and gender if documented in their chart. |
Comp | The most recent Comprehensive note for the current month. |
Additional MCP Notes | The most recent other encounter notes for the current month. |
Shift | The patient’s assigned shift. |
Time Left | The time the patient has left on dialysis. |
When the HHD, PD filter option is selected, a list of HHD (Home Hemodialysis) and PD (Peritoneal Dialysis) patients is displayed. The day-of-week and shift filters become disabled but the COVID-19 status filter is still enabled. If the modality-specific encounter note has not been created for the current month, the indicator is blank (no date) and has a +PD or +HHD based on the patient’s modality.
Column | Description |
Patient Name | The name of the patient. The information will be displayed as last name, first name, and will include the patient’s COVID-19 status, IKC badge, age, DOB, and gender if documented in their chart. |
Note | The most recent HHD or PD note for the current month. |
Patient’s Treatment Modality | The patient’s treatment modality type. |
Care team members enter the DaVita patient's COVID status into the Risk Event Management system (REM). If the patient meets the COVID-19 criteria, the COVID-19 status will show one of the three status tags: PUI Asymptomatic, PUI Symptomatic, or COVID-19 Positive. The COVID-19 status tags in the patient banner are color-coded: PUI Asymptomatic is yellow, PUI Symptomatic is orange, and COVID-19 Positive is red. Click the status tags to show the most recent status event information details. The information may include Status, Last Date in Facility, Facility Action, Receiving Dialysis in Facility, and Patient Treatment Going Forward. The status information is pulled from REM every hour.
The IKC(Integrated Kidney Care) badge appears in the patient banner for patients participating in an IKC program. The IKC badge will be displayed in the Dialysis Schedule and the Current Patients list. The IKC hospitalization risk rating appears in the patient banner, Dialysis Schedule, Current Patients, and IKC Patients tabs.
To open a patient’s chart, select the patient. Up to four patient charts can be open at the same time. To toggle back and forth between the patient tabs, click on the appropriate patient’s name. To return to the dialysis schedule, select Patients from the left side navigation menu.
To start an encounter note:
To start an encounter note, find the appropriate patient and encounters type. Click the blank indicator to create a new encounter. For ICHD patients, a new comprehensive encounter is created when the blank indicator is clicked in the “Comp” column. Click the indicator in the “Additional MCP Notes” column to create a new limited encounter for the ICHD patient.
The blue "+" icon is also available to create any encounter type. The most common note types for that modality appear at the top of the list, followed by the other note types. There is no limitation to the number of encounters that can be created from the Dialysis Schedule or Patient Chart.
NOTE: Non Visit notes do not appear on the Dialysis Schedule or the Monthly Visit Summary report. However, unfinalized Non Visit notes show in the In Progress Notes.
Selecting the appropriate note type for the patient will redirect you to the patient’s chart and starts an encounter note of the same type for editing. The screen will be split into two areas, Patient Chart on the left and Encounter Note on the right. Please refer to the CreateEncounter Note section under Patient Chart for full details about Encounter Notes.
The most recent encounters for the current month will display on the Dialysis Schedule. All encounter notes are available via the patient’s chart under the Clinical Summary tab in the Notes section.
Once the encounter note is closed by either finalizing or closing the encounter, you are returned to the Dialysis Schedule. Patients located next to the patient chart tab can also be used to return to the Dialysis Schedule.
Encounter notes that are In Progress (started or amended, but not yet finalized) display the encounter date and the indicator is highlighted in orange.
Finalized or Amended and Finalized encounter notes show the date of the encounter and have a green background.
To complete an encounter note that is in progress, find the appropriate patient and encounter type and click the unfinalized encounter. The pop-up menu appears with the note details at the top, including the note type, provider, and facility. Select “Edit” from the pop-up menu to open the encounter note. You can also view the PDF or deactivate the in progress note from this pop-up menu.
Finalized or Amended and Finalized notes show the encounter date and have a green background. Click the finalized encounter for the pop-up menu. The note details will appear at the top, including the note type, provider, and facility. The options for a completed note are Amend, Deactivate, or View PDF.
To deactivate an in-progress or finalized note:
STEP 1: Click the appropriate patient’s encounter note
STEP 2: Click Deactivate
STEP 3: Enter deactivation reason
STEP 4: Click DEACTIVATE button
To view PDF note:
STEP 1: Click the appropriate patient’s encounter note
STEP 2: Select View PDF
NOTE: A separate browser window will open with a PDF for that encounter.
View Previous Month
The “View Previous Month” toggle was added to allow providers to view the current and previous month’s encounter notes on the Dialysis Schedule. The toggle is available for ICHD and PD/HHD modalities. On the initial load of the Dialysis Schedule, the current month displays, and the toggle is turned off.
When the toggle is clicked, it turns blue, and the current and previous months are visible. Labels appear next to the note pills to designate the month. The current month displays on top, with the previous month underneath it. The same functionality/workflows are available for the current and prior months.
